Centrelake Imaging & Oncology is proud to offer Physicians direct access to patient reports and images. Reports become available the instant our Radiologists electronically sign the report. If you have multiple practices or multiple doctors in the same office, we can also link accounts so all patients can be seen in one list for easy management and tracking.
To begin the process, please download and complete this agreement form. Once complete, please contact your provider relations manager or send an e-mail to support@centrelakeimaging.com with "New Account" in the subject line.
Please note that signing up for portal access will not interrupt any services you currently receive to obtain reports by mail and/or fax.
